    Summary

    The purpose of this grant program is to provide funding and technical assistance to communities to address chronic disease risk reduction through evidence-based strategies and best practices that impact tobacco use, physical activity, nutrition and chronic disease self-management. All applications must address tobacco, while work in physical activity, nutrition and chronic disease self-management is optional.


    The grant program is structured to promote community program progress in two distinct phases:

    Planning - This phase is appropriate for applicants who lack a functioning coalition to support chronic disease risk reduction work. Planning applicants should not select work plans.

    • Participation in or creation of a relevant local community coalition. All CDRR grantees are required to engage a community coalition and/or a subcommittee of a larger community health coalition that focuses on commercial tobacco strategies.

    • Attendance at the annual Community Health Promotion Summit, two other CDRR approved trainings and three CDRR quarterly grantee meetings.

    • Development of a community chronic disease prevention plan based on community data available from existing sources. 


    Implementation - The CDRR implementation applicant must participate in a relevant local community coalition, at minimum for commercial tobacco related work plans. The coalition must provide meaningful, comprehensive support to guide and achieve the work plan objectives. Coalitions are essential mechanisms for policy, system, and environment change, as well as vitally important for ensuring equity and building capacity.     • Activities that further policy, systems and environmental change as outlined in the selected work plans will be reported on as instructed.

    • Functional chronic disease prevention coalition that meets at least quarterly. All CDRR grantees are required to participate in a community coalition and/or a subcommittee of a larger community health coalition that focuses on commercial tobacco strategies outlined in the work plans.

    • Attendance at the annual Community Health Promotion Summit, two other CDRR approved trainings and three CDRR quarterly grantee meetings.

    • Completed coalition self-assessment within three years or at the discretion of the Community Health Specialist. 


    State Fiscal Year 2025 proposals will be designed to make progress toward one or more of these goal areas:

    • Prevent initiation of commercial tobacco use among young people.

    • Eliminate nonsmokers' exposure to secondhand smoke.

    • Promote quitting among adults and young people.

    • Increase physical activity, access to nutritious foods, and community resiliency.

    • Promote development of diabetes workforce or prevent or delay type 2 diabetes.

    Additional Information

    Funds must be used for reasonable program purposes, including personnel, travel, supplies, and services. Funding restrictions include:

    • Clinical care (except as allowed by law) (Diabetes Prevention)

    • Costs not directly related to the CDRR grant

    • Defraying other costs associated with the treatment of diseases

    • Direct enforcement of policies

    • Direct patient care or rehabilitation

    • Direct services individual or group (e.g. tobacco cessation, nutrition counseling)

    • Fees for licenses or certifications (Diabetes Prevention)

    • Food and beverage items

    • Funds cannot be used for the preparation, distribution, or use of any material (publicity/propaganda) or to pay the salary or expenses of grants, contract recipients, or agents that aim to support or defeat the enactment of legislation, regulation, administrative action, or executive order proposed or pending before a legislative body, beyond normal, recognized executive relationships.

    • Furniture or unapproved capital equipment (computers, printers, trail counters) more than $500.

    • Grant writing

    • Medications, such as nicotine replacement therapy

  • Eligibility Details

    Any organization within the county or region can serve as the lead agency for the community. Organizational capacity to engage underserved populations and promote health should be considered.


    If the applicant is not a local health department, it is recommended that the initial application include a letter from the health department director stating that the department will serve on the coalition and support CDRR initiatives.


    Only one applicant will be awarded in a county. If more than one organization applies from the same community, those organizations may be matched to facilitate community collaboration and any award could be conditional upon agreement to partner. A consortium of counties may utilize a regional model approach and apply together under one application. 

    Deadline Details

    Project proposals were due mid March, 2024. The project period for this award is July 1, 2024 through June 30, 2025. A similar deadline is anticipated, annually.

    Award Details

    Award amounts vary. All applicants must provide a minimum of 25 percent match for every dollar awarded. The 25 percent match may be in cash, in-kind or a combination of both from county and/or public and private sources.
