    Summary

    The REACH Healthcare Foundation is implementing a five-year strategic plan with a revised theory of change that builds on the foundation's longstanding interest in health care coverage, access and quality. The strategic plan provides a sharper focus on improving the health of uninsured and medically underserved populations by emphasizing equity in health coverage and care. The Foundation's target population is uninsured people who live in the Foundation's service area. Additional emphasis is placed on highly vulnerable populations, including undocumented immigrant and refugee populations; homeless persons; and youth transitioning out of foster care.

    The Foundation will provide funding to support the following targeted outcomes:
    • Enroll all eligible people in health coverage;
    • Close the health coverage gap in Kansas in Missouri; and
    • Strengthen the safety net to serve those who remain uninsured and underserved.

    • Healthcare for individuals;
    • Medical or clinical research;
    • Programs focused on a single disease;
    • Endowment programs;
    • Operating deficiencies;
    • Substance abuse prevention and treatment;
    • Suicide prevention;
    • Domestic violence prevention;
    • Family violence prevention;
    • Equine or horticulture programs;
    • Senior services or health services for Medicare-eligible populations;
    • Crisis intervention programs;
    • Legal assistance to individuals;
    • Healthy lifestyles programs, such as nutrition education, fitness and food policy; or
    • Special events and fundraising activities

  • Eligibility Details

    Eligible applicants are nonprofit organizations and public agencies that benefit uninsured or medically underserved individuals in the following areas: Kansas City, Missouri, and Cass, Jackson and Lafayette counties in Missouri, and Allen, Johnson and Wyandotte counties in Kansas.
